Organizational Review and Strategic Planning

Assess to Improve

To help you determine your needs in client-centered care and employee empowerment, our Client Centered Review will provide you the template for change and growth needed to become a center of excellence. The process typically follows four steps:

1. Self-Review - Compare Yourself to the Best

Through our experience since our founding in 1982, we have developed assessment tools that will allow your organization to compare itself to the industry’s best practices in client-centered care and employee empowerment. Our questionnaires allow managers, staff, family members and residents to determine your organization’s needs. The results provide a depth and focus that identify very specific directions and the most appropriate steps to create centers of excellence.

2. On-Site Review - Assess Your Strengths, Limits and Needs

Once the questionnaires have been analyzed, FCS conducts an organizational review. The assessment targets these key areas:

Organization

A review is conducted on the overall operations of your organization, including team effectiveness, departmental integration, communication and staff recognition systems, staff empowerment and morale, managerial philosophy and effectiveness.

Environment

The functionality and personalization of the environment for clients and their families in the area of safety, practicality, privacy, esthetics and accessibility.

Care Process

The effectiveness of the clinical practices, documentation, process, admission process and supports, assessment mechanism, care consistency and effectiveness, communication of resident needs and idiosyncrasies.

Dementia Care And Behaviour Management

Assessment and programming tools, intervention and support strategies, medication use, ability to assess and integrate the clients’ past lifestyles and personal likes, client advocacy.

Customer Satisfaction

Family members and residents are interviewed to determine the degree of customer satisfaction, client involvement in decision-making, family involvement and community profile.

3. Strategic Planning - Plan Your Strategy for Change

Employees must have ownership in any changes that are implemented. A Towards Excellence Committee (TEC) is formed or an existing committee utilized. The TEC Committee is taught how to utilize the results from the self-assessment and on-site reviews and to develop and implement a plan of action with approval from the management team.

Committee members are trained to become effective change agents utilizing a formal change process to become departmental ambassadors responsible for soliciting recommendations, comments and assistance from staff in their own departments during implementation of the strategic plan. This process ensures ownership and involvement at all levels.

4. Follow-up Review - Assess Your Progress and Look to the Future

FCS conducts a follow-up review of progress to ensure that achievements are recognized, and to map out future longer-range goals. Through this process, your facility will move towards excellence in care and organizational dynamics, utilizing measures representing best practices in long-term care.
